Adult Day Health Care

A wide range of supportive health, psychological and social services are offered to aging, declining or disabled adults in a therapeutic, safe, fun, comfortable environment. Socialization, meaningful activity and peer and staff support decrease isolation, loneliness and boredom and enhance quality of life. Day health care can improve or maintain independence and physical, intellectual, emotional and mental functioning. Day health may enable primarily homebound adults to age in place at home with dignity, preventing or delaying nursing home or institutional placement, or facilitate transition from inpatient care or to or from a nursing home.

DDSN Program

Activity curriculums are specifically developed for Developmentally Disabled and Special Needs participants. An individual behavioral management program may be developed, to include activities to maximize behavior and intellectual functioning both at GC and home. Biofeedback is also available.

Physical and Occupational Therapy

Licensed Therapists may retrain in home and mobility skills, bed and chair transfers and use of environmental aids, such as walkers and canes.

Nutrition

A nourishing hot breakfast and lunch and nutritious afternoon snack designed by Certified Nutritionists are served daily.

Health Services

Nurses obtain medical recommendations and develop and update Individualized Plans of Care. Nurses administer and monitor medication and medical treatments and may coordinate other medical or health services.

Overall health and functional status assessment is ongoing. Participants are regularly monitored for early recognition and prevention of crises, and ambulation, toileting schedules, transfers and other Activities of Daily Living are also addressed.

Activities

Another focus is mental and spiritual needs. Planned activities, designed for individual and group participation, promote new interests and facilitate social interaction. Reality orientation, current events, memory challenges, word puzzles, music, crafts, games and movies stimulate intellectual activity and memory. Varying levels of physical exercise and movement, including Qigong, help meet individual needs and abilities. Guests entertain, and excursion destinations include sites of interest and shopping venues.

Transportation

We can provide transportation to and from a home. Drivers complete annual DOT physicals, National Certifications and recertifications in emergency procedures, including First Aid, CPR and AED. They and our vehicles excel on State-mandated electronic and on-site inspections.

Payment

There are several ways to pay - Medicaid, Long Term Care Insurance, Veteran's Administration, or self pay. In addition to self pay for Adult Day Health and In Home Health Care, possible supports for payment exist. Long-Term Care insurance plans may cover services. Although Medicare does not currently cover services, Medicaid does cover when a CLTC (Community Long Term Care) or DDSN (Developmentally Delayed and Special Needs) case worker refers for services. Veterans Administration often covers costs. Kindly reference the above "Resources" tab for additional possibilities for short-term or respite vouchers and grants that may pay for Adult Day Health and In home Heath Care services.
